"Aquende a neca e aperte os seios": inteligibilidade, trabalho e saúde nas experiências de pessoas trans universitárias / "Tuck the girldick and bind the breasts:" intelligibility, work, and health in the experiences of university trans people

Resumo As pessoas trans estão expostas a diferentes situações de vulnerabilidade que se entrecruzam, causando impactos negativos no processo saúde-doença-cuidado. Para melhor compreender essa questão, buscamos conhecer as experiências de sete estudantes trans de uma universidade pública de Minas Gerais, a partir de entrevistas norteadas por roteiro semiestruturado. As falas foram analisadas pela perspectiva da análise de conteúdo, e as categorias finais foram tensionadas com estudos das ciências sociais e da saúde coletiva. De forma geral, as narrativas se diferenciaram daquelas que compõem a maioria das pesquisas sobre o tema no Brasil, sobretudo em relação à manutenção de vínculos familiares e ao acesso à universidade pública. Nesse sentido, formulamos a hipótese de que a interação desses elementos com outros marcadores de diferenças pode contribuir positivamente para o estabelecimento de alianças e estratégias, a fim de lidar com o regime de inteligibilidade das identidades de gênero e com as dificuldades de acesso à educação, ao trabalho e à saúde. Compreender essa dinâmica e suas limitações, em uma perspectiva em que o individual se enlaça ao coletivo, possibilitou evidenciar o funcionamento da (hétero)normatividade nos processos de segregação de pessoas trans, assim como refletir sobre ações políticas que possam transformar essa realidade social efetivamente.

Abstract Transgender people are exposed to different situations of vulnerability that intersect, having negative impacts on the health-illness-care process. To better understand this issue, we sought to know the experiences of seven transgender undergraduates at a public university in Minas Gerais, from semi-structured interviews. The narratives were analyzed on the perspective of content analysis, and the final categories juxtaposed with studies of social sciences and public health. In general, the narratives are different from those which account for most of the research on the theme in Brazil, especially regarding maintaining family bonds and the access to public universities. Hence, we formulated the hypothesis that the interaction between these elements and other markers of differences can positively contribute to establishing alliances and strategies to deal with the regime of gender identity intelligibility and the constraints of access to education, work, and health. Understanding these dynamics and their limitations, under the perspective in which the individual and the collective are intertwined, enabled us to highlight the role of (hetero)normativity in the segregation processes of transgender people, as well as reflecting on the political actions that might effectively transform this social reality.

Nem essência, nem aparência: a produção de corpos, "normalidades" e liberdades mediadas pelas tecnociências / Neither essence nor appearance: the production of bodies, "normalities" and freedoms mediated by technosciences

Resumo Este artigo investiga em que medida certos corpos materializam as normas a partir de intervenções das tecnociências contemporâneas. Trata-se de um estudo qualitativo em que foram construídas três narrativas de pessoas que vivenciaram transformações corporais drásticas por meio de restrições alimentares, atividades físicas excessivas ou cirurgias bariátricas. Essas histórias foram tensionadas com literatura antropológica sobre o corpo e com as elaborações de teóricas feministas, especialmente Judith Butler e Donna Haraway, de forma a fomentar um debate a respeito dos efeitos individuais e coletivos da autoprodução de novas subjetividades. Evidencia-se, finalmente, que, no processo de materialização da "normalidade", as fronteiras entre a natureza e a técnica são borradas e os corpos podem se abrir tanto para novos constrangimentos como para novos projetos de liberdade em que nem suas supostas essências nem suas aparências estão mais em jogo.

Abstract In this article, we investigate to what extent some bodies materialize the norms from interventions of contemporary technosciences. It is a qualitative study in which three narratives of people who experienced drastic bodily transformations through dietary restrictions, excessive physical activities or bariatric surgeries were constructed. These stories were juxtaposed with anthropological literature about the body and with the elaborations of feminist theorists, especially Judith Butler and Donna Haraway, in order to foster a debate about the individual and collective effects of the self-production of new subjectivities. Finally, it is evident that, in the process of materializing "normality", the boundaries between nature and technique are blurred and bodies can open up to new constraints as well as to new projects of freedom, in which neither their supposed essences, nor their appearances are at stake anymore.

O cuidado da população LGBT na perspectiva de profissionais da Atenção Primária à Saúde / The care of the LGBT population from the perspective of Primary Health Care professionals

Resumo As pessoas LGBT se deparam com dificuldades e barreiras que prejudicam o acesso aos serviços de saúde. A falta de preparo e de sensibilidade dos profissionais, nesse contexto, são alguns dos elementos que reiteram as iniquidades em saúde e a vulnerabilidade desses corpos. Para conhecer esse fenômeno, entrevistamos 15 trabalhadores da Atenção Primária à Saúde e analisamos suas falas seguindo a perspectiva da análise do discurso foucaultiana. Os resultados evidenciaram que, apesar de os profissionais conhecerem a temática, eles usam estratégias discursivas que velam seus preconceitos e resistências, dificultando o reconhecimento das possibilidades de agência na transformação dessa realidade. A mobilização de categorias relativas às identidades e aos direitos sexuais em seus discursos indica deslocamentos no regime de sexualidade contemporâneo que devem ser considerados em intervenções de educação em saúde. A implicação de cada um na materialização das diferenças que marcam o campo sexual se demonstrou, nesse sentido, fundamental para discussão de formas de cuidado que sejam verdadeiramente acolhedoras e que não reforcem as desigualdades dos corpos que desafiam o binarismo e a heteronormatividade social.

Abstract The LGBT people face difficulties and barriers that hinder their access to health services. The lack of preparation and sensitivity of the professionals, in this context, are some of the elements that reiterate the health inequities and the vulnerability of these bodies. In order to understand this phenomenon, we interviewed 15 Primary Health Care workers and analyzed their speeches following the Foucauldian discourse analysis perspective. The results showed that although the professionals know the theme, they use discursive strategies, which veil their prejudices and resistance, making it difficult to recognize the possibilities of the agency in the transformation of this reality. The mobilization of categories related to sexual identities and rights in their discourses indicates displacements in the contemporary sexuality regime that should be considered in health education interventions. The implication of each one in the materialization of the differences that mark the sexual field proved to be, in this sense, fundamental to the discussion of care forms that are truly welcoming and do not reinforce the inequalities of bodies that challenge binarism and social heteronormativity.

Corpos trans: um ensaio sobre normas, singularidades e acontecimento político / Trans bodies: an essay on norms, singularities and political happening

Resumo Este ensaio pretende refletir sobre como a sexualidade tem sido circunscrita por alguns campos do conhecimento, sobretudo aqueles ligados à medicina, e como isso nos afeta no contexto social atual. Partindo do pressuposto de que estamos imersos em uma rede de dispositivos que se constituem como condição de nossa própria existência, examinamos especialmente as experiências relacionadas aos gêneros e às sexualidades que escapam do enquadramento normativo habitual, buscando localizar suas dimensões políticas a partir de alguns elementos extraídos das elaborações do psicanalista Jacques Lacan e dos filósofos Judith Butler e Giorgio Agamben. Pretende-se manter no horizonte a pergunta relacionada aos modos de resistência e invenções que podemos produzir estando dentro dessa rede. Adentramos na temática revisitando algumas noções incorporadas ao campo médico - sobretudo ao da psiquiatria - para, em seguida, nos debruçarmos, mais especificamente, sobre algumas experiências trans que têm ocupado espaço nos meios de comunicação de massa e redes sociais. Esses relatos, ao revelar algo que escapa a qualquer tentativa de normatização, subsidiarão propostas de formas do político, tendo como foco as singularidades com o objetivo de provocar reflexões que possam (re)orientar as nossas práticas no cotidiano.

Abstract This essay intends to reflect on how sexuality has been circumscribed by some fields of knowledge, especially those related to medicine, and how this affects us in the current social context. Based on the assumption that we are immersed in a network of devices that are a condition of our own existence, we especially examine the experiences related to genres and sexualities that escape from the established normative framework, seeking to locate its political dimensions from some elements extracted from the elaborations of the psychoanalyst Jacques Lacan and the philosophers Judith Butler and Giorgio Agamben. We intend to keep on sight the question related to the modes of resistance and inventions that we can produce being within that network. We approached the subject revisiting some notions that were incorporated into the medical field - especially that of psychiatry - and then, more specifically, we analyzed some trans experiences that have occupied space in the mass media and social networks. These reports, when revealing something that escapes any attempt of normalization, will subsidize proposals of forms of the political, focusing on the singularities to provoke reflections that can (re)orient our practices in the everyday life.

Sobre anorexias e bulimias: concepções e suposições etiológicas na perspectiva dos profissionais de Saúde / Sobre anorexias y bulimias: concepciones y suposiciones etiológicas en la perspectiva de los profesionales de Salud / On anorexia and bulimia: conceptions and etiological suppositions from the perspective of Health professionals

Anorexias e bulimias são síndromes graves que requerem um manejo cuidadoso. A prática clínica e a literatura mostram, entretanto, que sua abordagem é, muitas vezes, inadequada. Neste estudo, interessou-nos conhecer o discurso dos profissionais de Saúde acerca da caracterização e etiologia desses quadros. Foram realizadas entrevistas semiestruturadas com 13 trabalhadores da rede pública de saúde de um município de Minas Gerais. A partir dos pontos de convergência, divergência e silêncio das falas, localizamos eixos discursivos tais como distorção da imagem corporal, estigma e padrão estético do corpo magro veiculado pela mídia. Chegamos à formação discursiva de todos eles por meio de um diálogo crítico com a literatura médica, psicanalítica e sociológica. Os resultados encontrados apontam para a insuficiência do saber biomédico nesse contexto, indicando a necessidade de diálogos com outras teorias para o alcance de estratégias mais eficazes de abordagem.

Anorexias y bulimias son síndromes graves que requieren un manejo cuidadoso. No obstante, la práctica clínica y la literatura muestran que su abordaje es muchas veces inadecuado. En este estudio nos interesó conocer el discurso de los profesionales de salud sobre la caracterización y la etiología de esos cuadros. Se realizaron entrevistas semi-estructuradas con 13 trabajadores de la red pública de salud de un municipio de Minas Gerais. A partir de los puntos de convergencia, divergencia y silencio de los diálogos, localizamos ejes discursivos tales como la distorsión de la imagen corporal, estigma, estándar estético del cuerpo delgado mostrado por los medios. Llegamos a la formación discursiva de todos ellos por medio de un diálogo crítico con la literatura médica, psicoanalítica y sociológica. Los resultados encontrados señalan la insuficiencia del saber biomédico en ese contexto, indicando la necesidad de diálogos con otras teorías para alcanzar estrategias más eficaces de abordaje.

Anorexia and bulimia are serious syndromes that require careful management. Clinical practice and literature show, however, that their approach is often inadequate. In this study, we were interested in knowing the discourse of health professionals about the characterization and etiology of these disorders. Semi-structured interviews were conducted with 13 workers from the public health network of a city in Minas Gerais/Brazil. From the points of convergence, divergence and silence of the speech, we locate discursive axes such as distortion of the body image; stigma; the slim body aesthetic pattern conveyed by the media. We came to the discursive formation of all of them through a critical dialogue with medical psychoanalytic and sociological literature. The results point to the insufficiency of biomedical knowledge in this context, indicating the need for dialogues with other theories to reach more effective strategies of approach.

Psicanálise e Saúde Coletiva: aproximações e possibilidades de contribuições / Psychoanalysis and Public Health: approaches and possibilities of contributions

Resumo Este artigo discute possibilidades de contribuições da Psicanálise lacaniana para alguns impasses e dilemas vivenciados no campo de prática e pesquisa da Saúde Coletiva. Parte-se do pressuposto de que as formulações psicanalíticas potencializam transformações nas práticas em saúde relacionadas à desconstrução do paradigma biomédico, possibilitando uma abordagem mais complexa do processo saúde-adoecimento. Para isso, apresenta-se aspectos históricos, epistemológicos, teóricos, metodológicos e políticos através da problematização dos conceitos de inconsciente, sintoma, diagnóstico, transferência, trabalho interdisciplinar, produção do conhecimento e laço social. A reflexão pretende destacar a necessidade de se estimular reflexões criativas para além da mera prescrição de condutas, de forma a se operar em uma ética que foge da imposição de normas e valores. Propõe-se, assim, um alargamento da compreensão dos fenômenos, a identificação de novas dinâmicas e racionalidades, assim como a construção de novas formas de intervenção.

Abstract This article discusses possibilities of contributions from Lacanian Psychoanalysis to some impasses and dilemmas experienced in the field of practice and research of Public Health. It is assumed that the psychoanalytic formulations potentiate transformations in health practices related to the deconstruction of the biomedical paradigm, enabling a more complex approach to the health-illness process. For that, it presents historical, epistemological, theoretical, methodological and political aspects through the problematization of the concepts of unconsciousness, symptom, diagnosis, transference, interdisciplinary work, knowledge production and social bond. The reflection intends to emphasize the need to stimulate creative reflections beyond the mere prescription of conduct in order to operate in an ethic that escapes the imposition of norms and values. It proposes, therefore, an extension of the understanding of the phenomena, the identification of new dynamics and rationalities and the construction of new forms of intervention.
